Clinical Program Agreement

An Agreement between Trusted Neurodiagnostics Academy located at 211 Black Falls Rd, Dahlonega, GA 30533.  Main number is 404-857-2173 and  Stacey Jone (student) who resides at 535 Yale Avenue Galloway Nj 08205 (address).  Student's last 4 SSN: 5717 and date of birth 2/2/73

This Clinical Neurodiagnostic Program Agreement (“Agreement”) by and between Trusted Neurodiagnostic Academy (“TNA”) and the individual identified in the signature line below (“Student”), is effective the date indicated on the signature line below (“Effective Date”). Pursuant to this Agreement, TNA agrees to provide Student training in the use of neurophysiological testing (“Training”), subject to the following terms and conditions:

Term. The Course term and scheduling is subject to change at TNA’s sole discretion. 1494 clock hours

Scheduled Start Date March 2, 2026. Projected Graduation Date May 23, 2027

Projected Class Schedule:

Term 1: 3/2/26 -3/29/26              Term 4:  8/17/26 - 11/15/26

Term 2: 4/6/26 - 5/24/26            Term 5:  11/23/26 - 2/28/27

Term 3:  6/1/26 - 8/9/26             Term 6:   3/8/27 - 5/23/27

Required Supplies & Materials. TNA will provide the link to the textbooks and mannequin head for Student to order.  Invoice will be sent for the Study Guide.  Once paid, TNA will order the Study Guide.

    1. The online aspect of the training, as well as the textbook Rowan's Primer of EEG 3rd ed by Lara Marcuse, M.D., Madeline Fields M.D., Jiyeoun Yoo M.D.;
    2. The Handbook of EEG Interpretation 3rd ed by William Tatum, IV
    3. A Study Guide; and
    4. A mannequin head.

Student Requirements. 

    1. Train under a registered EEG Technologist for clinicals.  R EEG T. must be verifiable on ABRET®.
    2. Any internet or computer related expenses;
    3. Any additional facility credentialing costs;
    4. Any immunizations or tests necessary to complete credentialing;
    5. Background checks;
    6. General liability insurance; and
    7. BLS certification
    8. Any ABRET® related charges to sit for the R. EEG T® exam

Fees. Student agrees to pay TNA:

  1. Paying TNA $8000 tuition fifteen days prior to start date of first term; or
  2. If Student chooses to make payments, the total paid will be $8250.   In the event Student elects to make payments, Student must place a credit or debit card on file with PayPal, and authorizes PayPal to automatically withdraw such payments on the dates due.

Payment Options.

  1. Term Payment.  One of six payments totaling $1375 is due not later than the fifteenth day of the month prior to the beginning of each term.  
  2. Monthly Payments.  Monthly payments require the first payment of 550 due no later than the 15th of the prior month of the start of Term.  Monthly payments of $550 for the following 11 months payable via PayPal subscription.
  3. Bimonthly Payments:  Deposit of $500 due no later then the 15th of the prior month of the start of Term.  Bimonthly payments of $352.27 for the following 11 months payable via PayPal subscription.  Bimonthly subscription must be set up no later than the first day of Term.

Late Payments. In the event any payment due under this Agreement is returned as non- payable, or is otherwise not made when due, Student authorizes TNA to collect a $100 late fee and double payment on the date the next payment is due. TNA reserves the right to withhold Training from any Student not current on payments. Program tuition balance must be paid in full to receive a Certificate of Completion and be eligible to sit for the ABRET® R EEG T.

Collections.  In the event the Student cancels payment subscription or does not respond to collection efforts, TNA will utilize a collection agency and restrict the use of all TNA services.  TNA will email the Student an Intent to Collect notice prior to sending to collections.  Once the account is placed with the collection agency the account will be assessed collection fees up to 33.33% of the original balance.

Cancellations, Terminations, & Refund Policies.   A student may choose to withdraw from Trusted Neurodiagnostics Academy by officially notifying the Instructor prior to or on the date of the withdrawal from classes. Notification can be made by email, by Certified Mail or trackable delivery service, or by telephone.  Tuition is charged by term and is refunded by term.  Students are responsible for the payment of all completed or terms that have been started.

Cancellations:

  • Rejection of Applicant: If an applicant is rejected for enrollment, a full refund of all monies paid to the school must be made to the applicant, less an application fee of what the student paid.
  • Program Cancellation: If the school cancels a program subsequent to a student’s enrollment, the school must refund all monies paid to the school by the student.
  • Student Withdraws.  If a student withdraws or fails to attend class before the end of the first week, the student will receive a full refund of all monies paid to the school, less an application fee and required material fee of what the student paid.

Causes for Termination: A student’s enrollment can be terminated for the following reasons:

  • Failure to maintain Satisfactory Academic Progress as defined in the catalog;
  • Earning two (2) consecutive unexcused absences within one term;
  • Violation of the Academic and/or Student Conduct policies;
  • Failure to make payments in two consecutive installments; and/or
  • Expulsion from a clinical site.

Refund Policies:  Should a conflict exist between the school’s refund policy and the policy of a regulating agency of the state where the student resides or an accrediting agency, calculations for all refund policies will be performed and the student will be refunded the greater amount.

Refund Policy: Application fees will be refunded if the applicant requests a refund within three (3) business days after signing a contract; or if no contract is signed and prior to classes beginning the applicant requests a refund within three (3) business days after making payment. After that timeframe, if the student cancels, the application fee will not be refunded.

Partial Program Completion Refund Policy: Tuition is charged by the term and refunded by the term. Students are responsible for the payment of all completed and terms that have been started.

  1. Cancellation or termination after attendance has begun, will result in no refund for that term.
  2. Last Day of Attendance: In calculating the refund due to a student, the last day of attendance is determined by the last documented date the student submitted a graded activity within the LMS.
  3. Date of Determination: Refunds will be made within 30 days of termination of students’ enrollment or receipt of cancellation notice from the student

Academic Policies. Student acknowledges it has received a copy of the Trusted Neurodiagnostic Academy Clinical EEG Neurodiagnostics Program Academic Policies (“Policies”). Student has read, understands, and agrees to comply with the Policies.

  • Non-Completion. In the event Student fails to complete the Training, whether due to Student’s voluntary abandonment of the Training, being dismissed from the clinical site, the withholding of Training for non-payment, expulsion due to noncompliance with the Policies, or otherwise, the Student remains liable for, and is no way released from its obligations to pay the the Terms completed and the any Term which the Student has started.
  • Release of Liability: Student expressly releases and holds TNA harmless from any damages resulting from injury or exposure to blood borne pathogens while performing clinical hours in a student capacity. Student is responsible to inform TNA of any injury or exposure and to follow all facility protocols for said injury or exposure.
  • Media Release:  Unless this Student specifically emails [email protected] to decline the Media Release, the Student allows TNA to share the Student's success throughout the Program and Graduation.
  • Miscellaneous. This Agreement constitutes the entire agreement between the parties. There are no understandings, agreements, or representations (oral or otherwise) not specified herein regarding this Agreement. No waiver, consent, modification, or change of terms of this Agreement shall be binding unless in writing and signed by both parties. Each party, by signing this agreement, acknowledges that he/she/it has read this Agreement, understands it, and agrees to be bound by its terms and conditions. This Agreement shall be governed by the laws of the State of Georgia, and each party agrees to the exclusive venue and jurisdiction of the state or federal court located in Gwinnett County, Georgia for any action arising from this Agreement. In the event that any suit or action is instituted to enforce any provision in this Agreement, the prevailing party in such dispute shall be entitled to recover from the losing party all fees, costs and expenses of enforcing any right of such prevailing party under or with respect to this Agreement, including without limitation, such reasonable fees and expenses of attorneys and accountants, which shall include, without limitation, all fees, costs and expenses of appeals.
  • By Signing this Agreement.  By signing this agreement Student acknowledges that they have received and read the current school catalog.  The Student also acknowledges that they have read, understood, and received a copy of the completed and executed agreement.
